- ​​Chris and Kristen hold online office hours from 9 am to 5 pm Monday through Thursday.
- Patients and providers can call 701-989-4354 to ask questions and arrange     appointments.
​- Providers are also welcome to fax a referral form to us at 701-425-0104, ​and we can   contact the patient to schedule the visit.
